Registration Agreement
As an authorized representative of your organization or team, you understand and agree that your team and players attend the basketball tournament and use the facilities COMPLETELY AT THEIR OWN RISK. Your organization assumes full responsibility for all injuries and damages which occur in or about any events or programs on the premises of any facility hosting a LongIslandbasketball.com tournament, and your organization does hereby fully and forever release, discharge, and hold harmless LongIslandBasketball.com, all associated facilities and their owners, employees, and agents from any and all claims, demands, damages, or rights of action, present or future, resulting from any team's or individual's participation in any programs or use of the facility. Furthermore, LongIslandBasketball.com and its owners, employees or agents, shall not be held liable for any damage whatsoever arising from any personal injury or property loss sustained by any participant, or participant's family member, in or about any events or at any location. In addition, participant agrees to follow the rules of conduct and play set by LongIslandBasketball.com and the facilities it uses. Failure to do so may result in suspension from participation. CONSENT: I do hereby grant authority to the staff of Long IslandBasketball.com, or the staff of the facility where the event is being held, to render a judgement concerning medical assistance or hospital care in the event of an accident or illness if no parent or guardian is present. I do hereby authorize LongIslandBasketball.com and its assigns to utilize any and all photographs, pictures or other likeness of me or anyone assigned guardianship to me, as they deem appropriate in its promotional materials or team films.
